The Community Resource Team is an integrated Health and Social Care support servise that provides a range of professional health and social care responses to enable people to achieve what matters to them.

We have a multidisciplinary approach to enabling and suporting people who live with issues caused by disability, frailty and ageing.
You will be part of a short term social work reponse within the Community Resource Team and within secondary care provision in Bridgend County Borough, delivering social work support to adults in an integrated multi-disciplinary team comprised of social work, social care and health professionals; the aim of the team is to improve access to health, social care and personal assistance services for people affected by frailty and ageing. This service focuses on outcomes for individuals, maximising individual potential and minimising risk to independence, with an emphasis on choice and control. We have worked with partners to develop integrated provision which is person centred and this post will have a key role in working to ensure that people in hospital are dischaged in a timely manner. As a member of the Community Resource Team you will share with other senior staff the responsibility for the effective operation of the service and participate in its ongoing development.
A recognised social work qualification and registration with the Care Council for Wales is essential.
You will:-Work as part of a team in a climate of change.-Manage a caseload in a busy environment -Provide advice and guidance and signposting people into community resources where appropriate and necessary. -Promote holistic wellbeing outcomes for people who need care and support -Develop professional relationships within the Directorate, Health Board and across agency boundaries;-Take a person centred approach to practice;
Protecting children, young people or adults at risk is a core responsibility of all council employees.
