We support people to maintain their independence, regain, improve and maintain skills of daily living, provide the best outcomes for people who need care and support in a friendly and safe environment. You will work as part of a social care team. You will work closely with District Nurses, Social Workers, Therapists and other professionals across health and social care sector.
We are looking for people with sound values, respectful, able to contribute and make a difference to people’s lives. Be a good listener and have good communication skills and are able to work as part of a professional multi skilled team.
