23 hours per week
This is an exciting time for a support worker to join our energetic and committed joint Health and Social Services multi- disciplinary team based within the Community Care Resource Team, at Trem-Y-Mor Resource Centre in Bettws.
The Community Resource Team comprise of the following: Acute Clinical Team and Mobile Response Service, Bridge link Telecare Service, Reablement, Enabling Homecare, Community Independence and Wellbeing Team, Community Occupational Therapy and Integrated Community Equipment Service.
The aim of the Mobile Response Team is to support people to remain as independent as possible in their own home. Providing an Early Response Service linked to the Nurses/Therapists within the team, providing rapid assistance for people in need in the community, responding to the immediate personal care needs of older people and those with a disability in their own homes.
You will also react to calls for assistance from Telecare users via the community alarm centre.
Working as part of a team providing a 24 hour service 7 days a week, you will be required to work weekends, waking night shifts for which you will receive a time and third payment for. You will also be required to be on the emergency response standby rota for which you will receive an additional payment for.
If you have an understanding of the needs of older people and people with disabilities, can remain calm in an emergency situation and act on your own initiative, we would like to hear from you. A full driving licence is essential for this post.
