An excellent opportunity has arisen for a Personal Advisor to join Bridgend’s 16+ service. The service provides support to young people aged 16 to 25 who are currently looked after, and those who have ceased to be looked after, to successfully make the transition into adulthood. You will have knowledge and understanding of the Social Services and Wellbeing Act (2014) and the associated responsibilities of the role of the Personal Advisor which include support relating to matters of accommodation, income, health, education, training and employment. Working in conjunction with other professionals, including foster carers, residential and supported accommodation staff, education, housing and health professionals, you will be responsible for reviewing and implementing Pathway Plans, whilst supporting the development of the independence, and self-care skills, of young people. You will work closely with the Social Worker’s within the 16+ Team when the young person is looked after until the age of 18 and then you will be primary worker for the young person when they reach adulthood.We are seeking an experienced, and highly motivated person, who is committed to significantly improving the life chances of young people and promote positive outcomes. You will have a relevant qualification and/or a proven track record in successfully working with young people.
