Bridgend County Borough Council is seeking to appoint a Social Work Team Manager into its Integrated Cluster Network Team in the North Cluster Network area of Bridgend; this is an exciting opportunity to join and lead the Social Work element of the Integrated Cluster Service.
We are seeking to appoint a dynamic experienced registered senior social worker to lead the social work team; you will be a practitioner who understands and is committed to integrated, multidisciplinary approaches to practice. As well as managing the the Social Work element of the service, this is a key leadership role in a fully integrated service, working in an MDT model with a strong focus on strength base practice, choice, voice and control, as well as care and support closer to home.
As the Social Work Team Manager you will be responsible for ensuring and delivering high standards of social work practice, as well as working within an integrated multiprofessional approach to offering support to individuals, their families, and carers to achieve their goals and remain independent; working mainly, but not exclusively, with older adults and those affected by frailty, chronic ill health and disability.
The professionals in the multidisciplinary team draw on the skills of District Nursing, Physio and Occupational Therapy, CPN, Pharmacy, Speech and Language Therapy, Dietetics, and Wellbeing workers, to give holistic assessment and support to adults living within the Cluster area. In addition, we have strong relationships with our colleagues in the Third Sector, working together to support individuals, their families and carers to maintain their independence and lead active lives within their local communities.
