The Common access point is a busy, office based access team for the Community Integrated Services in Bridgend. The team provides a one stop information, advice, signposting and referral service for adults.
You will be a skilled communicator with the emotional intelligence required to undertake sensitive ‘What Matters’ conversations with individuals and carers who are eligible for an assessment under the Social Services and Wellbeing Act 2014.
You will be part of the Community Resource Team, delivering an integrated approach to people affected by issues of disability, frailty and ageing. You will work with multi-disciplinary teams comprised of social work, social care and health professionals; the aim of the service is to improve access to health, social care and support services for people affected by, long term chronic and or acute health conditions, frailty and ageing. The service focuses on outcomes for individuals, maximising individual potential and minimising risk to independence, with an emphasis on choice and control. We have worked with partners to develop an integrated provision which is person centred and this post will have a key role in contributing to the access and delivery of these services.
Protecting children, young people or adults at risk is a core responsibility of all council employees.

The ability to greet customers through the medium of Welsh is a requirement for this post.
