The Community Resource Team is a joint Health and Social Services multi- disciplinary team based within Intermediate Care in Bridgend County Borough.
We have aligned a range of Intermediate Care and community services to develop the Community Resource Team which supports individuals to maximise their independence in their home environment.
The Community Resource Team (CRT) currently comprises of the following: Early and Mobile Response Service, Telecare, Residential and Community Reablement, an Enabling Homecare Service, Community Independence & Wellbeing Team and the Integrated Community Equipment Service.
You will be a key part of the CRT service which aims to take an integrated approach to services that support people to continue to live in their chosen environment by preventing avoidable hospital or care home admission, and work closely with hospital discharge services to support discharge home.
Your role in the CRT will be to carry out multi-disciplinary assessments, working with individuals to identify and mitigate the risks to their independence, as well as with their families and carers.
As an experienced (at least 2 years post qualifying) member of the Community Resource Team you will share with other senior staff the responsibility for the effective operation of the service and participate in its ongoing development.
