Our Supported Living Services are based across the borough of Bridgend and provide support to people with learning disabilities who live in their own homes. The role of the Team Leader is to manage services and ensure the ongoing development and delivery of individualised support.
The Team Leader will manage the day to day services of 2-3 supported living settings in localised, geographical areas.
The service supports a diverse group of people with a range of needs such as Autism, complex behaviours, physical disabilities, communication difficulties, Dementia and complex health needs. Therefore experience and knowledge of working with a diverse group of individuals is an essential element of the role
Duties and responsibilities relating to staff include recruitment, selection, training and development, and planning required with rota management. Supervision and appraisal of staff will be an expectation of the role as will 'on site' role modelling and observational practice.
The role will be critical to the reshaping of our support model and will involve the Team Leader assisting to develop services that move from a 'care' model to an individualised, enabling model of support. They will need to implement positive risk management approaches and evidence that individuals have been supported to achieve desired outcomes
The Team Leader will be responsible for the 'first line' monitoring and administration of personal and organisational finances / resources / budgets, Housing Management
Good communication skills are required as they will be the main contact with Care Management, Health Professionals, generic services, families and carers and internal departments such as H.R, Training and Finance
