The Community Resource Team supports individuals to maximise their independence in their home environment. The Team comprises of the following: Early Response/ Mobile Response Service, Telecare, Reablement, Bridgestart 'Enabling Homecare', Community Independence and Wellbeing Team and Integrated Community Equipment Service.
You will be a key part of the CRT BridgeStart short term enabling service which aims to support people to maximise their independent living skills and continue to live in their chosen environment also preventing avoidable hospital or care home admission or where this is required to support discharge home.
You will work in an enabling way, promoting independence under the direction of the BridgeStart Occupational Therapist and Team Leader. You may also be called upon to cover other Community Resource services as needed.
The service currently operates 7 days a week, 365 days a year, mainly between the hours of 7.30am and 10.00pm. As part of a team your working pattern will include some weekends and bank holidays.
You will need to be flexible and committed and prepared to undertake appropriate training, including QCF level 2/3 in Health & Social Care. You will hold a full driving licence and have the use of your own car.
