We are seeking to appoint a registered social worker for up to 2 years, to be part of an Integrated Health and Social Care Community Network. You will work with adults with a variety of needs requiring an assessment, care management and social work service.
Carrying out high quality assessments of the strengths, needs and risks to independence of adults, as well as their carers, you will work in partnership with other agencies and professionals in a co-ordinated manner, to address the outcomes identified by individuals.
We offer regular supervision, encourage professional development and provide a supportive environment.
For the above post a recognised social work qualification e.g. BSc (Hons) Social Work, C.Q.S.W., C.S.S., Dip S.W. and registration with the appropriate regulatory body is essential.
