We are looking for a highly skilled, creative and innovative Social Work Team Manager to lead and manage a team of social workers, focussing on the needs of adults over the age of 65, supported through secondary mental health.
The successful applicant will lead on excellent social work practice, ensuring that the person’s voice is always heard and respected. They will form key partnership relationships, working closely with clinical colleagues and also Bridgend’s Integrated Network Service. They will also take a key role in the development of the Integrated Complex Dementia Day Assessment Service. The Social Work Team Manager will focus and well-being and independence goals, for people they support and those important to them. They will have a specific focus on hospital avoidance; strength-based practice and working to the aims of the Dementia Pathway of Standards in Wales.
