The Community Resource Team supports individuals to maximise their independence in their home environment by delivering a range of intermediate care and community services.
The Community Resource Team comprises of the following: Acute Clinical Team, Telecare & Mobile Response Team, Community Reablement, a Reablement Unit, Early Intervention and Prevention hub, Sensory Team, Speech and Language, Pharmacy and the Integrated Community Equipment Service.
You will ensure individuals in receipt of the Bridgelink Telecare Service are using the most appropriate and effective TEC/ telecare equipment to meet their changing needs. This will be achieved by working with prescribers, internal stakeholders and those external services commissioned by the council to deliver aspects of the TEC/ Telecare service.
You will develop and maintain a comprehensive knowledge of Telecare and Technology Enabled Care (TEC) equipment, its use, application, and effectiveness. You will advise professional staff on its application and review existing clients within the Telecare service to ensure that their equipment appropriate and supports independent living- extending the length of time people are able to live at home.

Requirements:
Up to date and good knowledge of digital technologies or a willingness to learn. Well-developed communication skills.
Experience of working in a fast-paced office environment, good I.T. skills and previous experience of using office systems.
Working in a client-driven environment, you will have excellent communication skills and an ability to work to strict timescales and meet a variety of deadlines.
The ability to greet customers through the medium of Welsh is a requirement for this post.
Protecting children, young people or adults at risk is a core responsibility of all council employees.
An Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
A valid driving licence is requirement for this post.
