We provide a range of care and support services, and you will play a vital role helping people to live as independently as possible in their own homes in our communities.

Your key responsibilities will include: -
Providing person centred support tailored to individual needs, recognising strengths, capabilities and desired outcomes. Encouraging people to make informed choices recognising what matters to them in their lives.
Providing support to promote independence with aspects of daily living as identified in individuals care and support plans which may include diet/food preparation, assistance with medication and personal hygiene.
To work as part of a team alongside social workers, therapists, and health colleagues to meet identified outcomes.
Participating in supervision sessions and career development opportunities as agreed with your manager and actively engage in training in order to maintain your own professional development and registration with Social Care Wales and enhance competency within your role.
To be aware of your responsibilities safeguarding individuals at risk and report any concerns which may place the individual, you, or others at risk.
