We are seeking a motivated, enthusiastic, and committed individual to join our established Support at Home Team. This role combines hospital discharge planning with the coordination of community referrals into Short-Term Assessment and Reablement Services, supporting people to live safely and independently at home.
About the Role
Working closely with individuals, families, health professionals, social care providers, and community partners, you will:
Support safe and timely hospital discharges using the Discharge to Recover and Assess model.
Coordinate community referrals into Short-Term Assessment and Reablement Services to promote independence and prevent unnecessary hospital admissions.
Work collaboratively with multidisciplinary teams to ensure coordinated, person-centred support
Monitor and manage referrals, assessments, and service capacity to ensure timely interventions.
Identify risks associated with discharge and community living arrangements.
Maintain accurate records and contribute to the continuous improvement of the service.
About You
We are looking for someone who:
Has experience within health, social care, reablement and community services
Is an excellent communicator with strong organisational skills.
Can build positive relationships with individuals, families, and professionals.
Is able to prioritise workload effectively in a fast-paced environment.
Has knowledge of person-centred practice, safeguarding, and relevant social care legislation.
Is committed to delivering high-quality services that promote independence and wellbeing.
